Check Audio MIDI Setup (Applications/Utilities/Audio MIDI Setup) to verify that the Focusrite Rednet PCIe card is set to 128ch 32-bit Float 48.0Khz for both input and output.   

This setting can revert to 64 channels especially if new audio drivers are installed.

If this occurs it will affect both input and outputs. The outputs affected would likely be re-renders and headphone outputs.  

If the problem persists delete the device and system preferences, reboot and reset. Navigate to MacintoshHD/Library/Preferences/Audio and delete both:

    com.apple.audio.DeviceSettings.plist

    com.apple.audio.SystemSettings.plist

Note that this will reset preferences for any other installed audio devices.
